Youth at Risk - Information and Support: Leaving Care
A program for young people post DHHS care. When a young person turns 18, they are able to live independently of DHHS, a time where additional support is often needed for full independence to be achieved.
The young person is allocated a case manager, who assists then in reaching their independent living skills
